- Responsible for providing comprehensive primary care, coordination of 24-hour care, medical coverage, and directly provides services to ArchCare participants.
- Provides face-to-face comprehensive History and Physical assessments on new referrals. Prepares face-to-face Quarterly or as needed History and Physical assessments on established participants.
- Provides evaluation and treatment of participants with acute illnesses.
- Participates in the development and revision of the participant's plan of care as a member of the interdisciplinary team. Participates in discharge planning.

Responsibilities
The Clinical Nurse Manager is responsible for assisting in the implementation of home health clinical services within the Certified Home Health Agency (CHHA) in a defined area in compliance with applicable laws, regulations and Agency policies.
